The reluctance to rev and the white smoke suggest that you have lost compression on 1 or more cylinders, this could be down to a head gasket failure and the white smoke being coolant.
Have you checked the coolant level itself for loss and oil contamination in the expansion vessel etc?
If it was petrol engine i would suggest that you whip out the spark plugs and check each cylinder for compression or lack of it but as its a diesel i dont know if you took out the glowplugs that you couldnt do the same.
if compression, coolant/oil levels are ok then it could be more turbo based but i would have thought that the smoke would have been black from fuel or oil etc.
hope this helps.

right so oil and coolant levels are ok and it revs freely to 4k but then dies.
I would be looking down the route of the the turbo side of things.
!) check the air filter/assembly and all associated hoses upto the turbo (cold side) then check the pressure side of the turbo to the intercooler to the inlet manifold (hot/pressure side) for leaks/splits in hoses or clips that have failed.
2) If thats all ok then the 4k cut out may be some kind of limp homemode that protects the engine in cases of wastegate failure or over boost situations.
3) Then there is the physical condition of the turbo charger, I would pop off the inlet hose to the compressor housing on the turbo and check the impellor wheel for excessive free play not only laterally but also back and forth.
it sounds like the car needs a diagnostic computer connecting to see what codes are flashed up when the problem arises.
I would bite the bullet and pay for a diagnostic session so atleast you will be batting in the right area.
sorry i cant be anymore help as although im very familiar with turbocharged setups they are all of the older mechanical type on audi quattro's and so are far more basic.
